There is never a single cause for an incident and is usually a series of events. This seminar is designed to give the participants an insight in the complexity of Accidents and Incidents and the methods for prevention and reducing both Accidents and the resulting costs. This requires a detailed investigation into why circumstances have caused machinery to break down and even to have caused a physical injury.


The course is designed to primarily and specifically enhance Maintenance and Reliability Operations and to positively decrease losses in equipment downtimes by effective process of logical investigations processes RCFA system for Analysis of Cause and Effects. This course includes a variety of accurate cause and effects tools for RCFA Analysis and Decision Making functions.
